Can you exercise on Mounjaro, Wegovy or Ozempic?
Yes, and the medicines are meant to be used that way. The US prescribing information for Wegovy (semaglutide) states that it is indicated in combination with a reduced calorie diet and increased physical activity, and the tirzepatide (Mounjaro) trial participants had diet and activity counselling too. What a label does not tell you is what kind of exercise, how much, or how to train on a day when you feel sick. This guide is for adults whose doctor has prescribed one of these injections and cleared them to exercise.
Whether a medicine is right for you, and every decision about its dose, timing or stopping, belongs to the prescribing doctor. India's Ministry of Health and Family Welfare said on 24 March 2026 that these drugs, when used without proper medical supervision, may lead to serious adverse effects, and advised citizens to use them only under the guidance of qualified medical practitioners. Nothing here is advice to start, stop or change a medicine.

How much muscle do you lose on Mounjaro or Wegovy?
In the two main obesity trials, roughly a quarter to two-fifths of the weight lost was lean mass rather than fat. Both trials scanned a subgroup with dual-energy X-ray absorptiometry (DXA), which separates fat from lean tissue.
| Trial and length | Weight change | Fat mass change | Lean mass change |
|---|---|---|---|
| STEP 1, semaglutide, 68 weeks (140 people scanned; 95 on the drug) | −15.0% (placebo −3.6%) | −19.3% | −9.7% |
| SURMOUNT-1, tirzepatide 5, 10 or 15 mg pooled, 72 weeks (160 scanned; 124 on the drug) | −21.3% (placebo −5.3%) | −33.9% (placebo −8.2%) | −10.9% (placebo −2.6%) |
As shares of the weight lost: the 2025 joint advisory on nutrition for GLP-1 therapy (Mozaffarian et al.) reports that in STEP 1, of an average 13.6 kg weight reduction, 8.3 kg (62%) was fat mass and 5.3 kg (38%) was lean body mass. In SURMOUNT-1, Look and colleagues (Diabetes, Obesity and Metabolism, 2025) found that approximately 75% of the weight lost was fat and 25% was lean mass, for both tirzepatide and placebo. Across all 1,961 STEP 1 participants (Wilding et al., NEJM 2021), mean weight change at 68 weeks was −14.9% with semaglutide and −2.4% with placebo, both alongside lifestyle intervention.
Keep these numbers in proportion. Lean mass is not all muscle: it includes water, organs and other non-fat tissue, and the advisory estimates muscle is about half of it, so roughly 20% of the weight lost in STEP 1 was muscle. Fat fell faster than lean tissue, so in STEP 1 the share of lean mass in the body rose by 3.0 percentage points. And the advisory notes similar losses after bariatric surgery and very low-calorie diets. The losses are not unique to the drugs, but they are real.
In SURMOUNT-1, participants were counselled towards a 500 kcal a day deficit and at least 150 minutes of activity a week, without a specific strength training protocol. So the figures show roughly what happens without structured lifting.
Why does keeping muscle matter when you lose weight fast?
Muscle lets you climb stairs, carry shopping and stay steady on your feet as you age. The joint advisory lists the consequences of low muscle and bone mass as physical impairment or disability, falls and fractures, surgical complications, reduced quality of life and decreased survival. It also cites modelling suggesting that, without structured strength training, muscle may account for 10–15% of the weight lost in women and 20–25% in men.
The STEP 1 investigators wrote that weight loss reduces lean muscle mass, potentially affecting resting energy expenditure and physical functioning. A 2017 review in Advances in Nutrition (Cava, Yeat and Mittendorfer) is reassuring on one point: diet-induced weight loss reduces muscle mass without adversely affecting muscle strength, and physical function improves as fat comes off. It also warns that losing lean mass could raise the risk of sarcopenia, low muscle mass with impaired function; the joint advisory notes these losses are a bigger concern at older ages. Women over 40 can read more in strength training for women over 40.
Does strength training on GLP-1 drugs protect muscle?
The evidence says it helps, though most of it comes from weight loss by diet rather than from trials of the injections themselves. The joint advisory states that structured strength training, or mixed strength and aerobic training, is well established to help preserve lean mass during weight reduction, and that aerobic activity alone has a smaller effect when weight comes off quickly. The Cava review found that both endurance and resistance exercise help preserve muscle mass during weight loss, and that resistance exercise also improves muscle strength.
The American College of Sports Medicine's position stand on physical activity for weight loss (Donnelly et al., 2009) says resistance training does not enhance weight loss but may increase fat-free mass and increase loss of fat mass, and is associated with reductions in health risk. Lifting will not make the scale move faster, but it may change how much of what you lose is fat. The advisory also describes a randomised trial in which a year of GLP-1 therapy combined with exercise preserved bone mineral density, while the drug alone decreased it.
The advisory's recommendation: GLP-1 medicines should be prescribed together with a structured exercise program, aiming for strength training at least three times a week plus at least 150 minutes of moderate-intensity aerobic exercise a week, customised to the person's fitness. The plan below builds up to that.
Food is the other half. The advisory notes that targets of 1.2–1.6 g of protein per kg a day have been proposed during active weight reduction, that it is unclear which body weight to use for people with obesity, and that a fixed 80–120 g a day may be easier to follow. With a small appetite that takes planning; see protein on weight-loss injections. For Indian food values, see protein in Indian food, and get a personal number from the protein intake calculator.
What should a 12-week strength plan on Mounjaro or Wegovy look like?
This plan assumes low appetite and some tired days. It starts at two short sessions a week and reaches three by week 5. Each session works the whole body: a squat, a hip hinge, a push, a pull and a carry or core hold. At home, a chair, dumbbells or a resistance band is enough. Our dumbbell workout at home and resistance band workout guides show the exercises, and the gym workout plan for beginners explains the machines.
- 1
Weeks 1–2: two sessions, learn the movements
Twice a week, 25–30 minutes. One or two sets of 10–12 reps per exercise, stopping with at least three reps left in reserve. Gym: leg press, Romanian deadlift with light dumbbells, machine chest press, seated cable row, plank. Home: sit-to-stand from a chair, glute bridge, wall or incline push-up, band row, dead bug. Walk on two other days.
- 2
Weeks 3–4: same two sessions, a little more work
Move to two sets of every exercise. When all reps feel steady, add the smallest weight step available or a slower lowering phase at home. Keep sessions to about 30 minutes.
- 3
Weeks 5–8: three sessions a week
Add a third session on a non-consecutive day so you reach the advisory's three times a week. Rotate two workouts: A (squat, push, pull, core) and B (hinge, lunge or step-up, overhead press, row, loaded carry). Two to three sets of 8–12 reps. On a low-energy day, do the first two exercises only and call it done.
- 4
Weeks 9–12: progress the main lifts
Keep three sessions. Each week, try to add one rep or a small amount of weight on the squat, hinge, push and pull. Write down what you lifted: strength holding steady or rising while the scale falls tells you your training is working.
- 5
Every week: the aerobic minutes
Build walking or cycling towards 150 minutes of moderate effort a week, the level where you can talk but not sing. Split it into 20–30 minute walks. Our walking for weight loss guide has a progression.
- 6
Every 4 weeks: review with numbers
Check your lifts, waist, weight and energy. If strength is dropping or you are struggling to eat, raise it with your doctor or dietitian rather than training harder.
| Day | Session | Time | On a bad day |
|---|---|---|---|
| Monday | Strength A at the gym or at home | 40–45 min | First two exercises, then a slow walk |
| Tuesday | Brisk walk | 30 min | 15 min easy walk |
| Wednesday | Strength B | 40–45 min | Seated or supported versions only |
| Thursday | Brisk walk or easy cycling | 30 min | Rest |
| Friday | Strength A | 40–45 min | One set of each exercise |
| Saturday | Longer walk | 45–60 min | 30 min |
| Sunday | Rest, light stretching | As you like | Rest |
That week totals three strength sessions and about 105–120 minutes of walking; add a 10–15 minute walk after each strength session to reach about 150. If you notice your side effects follow a pattern through the week, put the walks and rest on your worst days and the strength sessions on your best ones.
Can you work out with nausea, dizziness or low energy on these injections?
Often you can, at a lower effort, but not if you are vomiting, dizzy or unable to keep fluids down. The NHS lists feeling or being sick, constipation, diarrhoea and feeling tired among the common side effects of both semaglutide and tirzepatide. In STEP 1, nausea and diarrhoea were typically transient and mild to moderate, and subsided with time. The joint advisory adds that these side effects are more likely when the drug is started or the dose is increased, and that nausea often comes in the morning or after long periods without eating.
- Nausea. Train after a small meal or snack, not on an empty stomach or straight after a big meal; the advisory notes vomiting is more likely with large meals. Swap jumping, fast floor-to-standing moves and head-down positions for seated and supported exercises.
- Fluids. The Wegovy Medication Guide warns that diarrhoea, nausea and vomiting may cause dehydration, which may cause kidney problems, and says it is important to drink fluids. Sip water through the session and skip training on a day you cannot keep fluids down.
- Dizziness. The NHS advises not to drive, ride a bike or use machinery until dizziness has gone. Treat a treadmill or exercise bike the same way, and stand up slowly from the floor.
- Low blood sugar. The NHS says it is more likely if these drugs are used with other diabetes medicines. If that applies to you, ask your doctor how to check your sugar around exercise, and stop at the first sign of shaking, sweating or confusion.
- Heart rate. The Wegovy label says it can increase your resting heart rate. Tell your doctor if your heart races or pounds for several minutes, and judge effort by breathing and talking rather than a watch target.
- Tiredness. Use the bad-day column in the table. A short session keeps the habit going.
