Why we ask
Your coach guides you over video and can't physically help you, so knowing about your health before you start is how we keep your training safe. This declaration forms part of our Terms & Conditions, and you accept it when you book a session or plan.
Questions to answer before your first session
These questions are based on internationally used pre-exercise screening. If you answer yes to any of them, talk to your doctor before your first session, and tell your coach.
- Has a doctor ever said you have a heart condition or high blood pressure?
- Do you feel pain in your chest at rest, during daily activities or when you exercise?
- Do you lose your balance because of dizziness, or have you lost consciousness in the last 12 months?
- Have you been diagnosed with another long-term condition, such as diabetes, asthma, or a lung, kidney or liver condition?
- Do you take prescribed medication for a long-term condition?
- Do you have a bone, joint or soft-tissue problem (muscle, ligament or tendon) that could get worse with exercise?
- Have you had surgery, an injury or a fracture in the last 6 months?
- Are you pregnant, or have you given birth in the last 12 months?
- Has a doctor ever said you should only exercise under medical supervision?
Your declaration
By booking a session or plan, you confirm that:
- the information you give us about your health is true and complete, as far as you know;
- you've gone through the questions above and, if you answered yes to any, you have your doctor's approval to exercise or will get it before you train;
- you'll tell your coach about any new injury, illness, pregnancy or change in medication before each session;
- you'll train in a safe space, at your own pace, and stop and tell your coach if you feel pain, dizziness, chest discomfort, unusual breathlessness or unwell;
- you understand that exercise carries some risk and that your coach guides you remotely and can't physically help you;
- you'll follow your coach's instructions and won't attempt exercises beyond your ability or that your coach hasn't set.
Your consent to use your health information
You agree to us collecting and using your health information, meaning your answers to the booking questions, what you tell your coach and your coach's session notes, only to plan and run safe sessions for you. It's shared only with your coach and the team members who need it, and it's never used for marketing. Our Privacy Policy explains how we protect it.
You can withdraw this consent at any time by writing to hello@makemyfitness.com. Without it, we can't continue to coach you safely.
Booking for someone under 18
If you're a parent or guardian booking for someone under 18, you make this declaration and give this consent on their behalf, and you agree to supervise their sessions.
In an emergency
Keep your phone within reach while you train. When we confirm your booking, tell us your address and an emergency contact, so your coach can help get assistance to you if something goes wrong.
If you feel chest pain, severe breathlessness or faint, stop immediately and call 112.
