How much protein do you need on weight-loss injections?

There is no single agreed number, but the clearest guide so far is the 2025 joint advisory on nutrition for GLP-1 therapy from the American College of Lifestyle Medicine, the American Society for Nutrition, the Obesity Medicine Association and The Obesity Society. It notes that higher targets of 1.2–1.6 g of protein per kg a day have been proposed during active weight loss, and that an absolute target of 80–120 g a day may be easier to follow while still covering needs.

The hard part is which body weight to multiply by. The advisory says it is unclear whether targets for people with obesity should use actual, adjusted or ideal weight, or fat-free mass, and that using actual weight can significantly overestimate needs. It also sets two limits: protein should not fall below 0.4–0.5 g/kg a day, and prolonged intake at or above 2 g/kg a day should be avoided. For comparison, the ICMR-NIN Dietary Guidelines for Indians (2024) set the general RDA at 0.83 g per kg a day, or 54 g for a 65 kg adult.

When should you call your doctor instead of changing your diet?

Mounjaro, Wegovy and Ozempic are prescription medicines. Whether one is right for you, at what dose and for how long, is your prescribing doctor's decision. In March 2026, India's Health Ministry said these drugs, used without proper medical supervision, “may lead to serious adverse effects”, and advised people to use them only under the guidance of qualified medical practitioners. Separately, the ICMR-NIN Dietary Guidelines for Indians (2024), written as general advice for the public rather than as guidance on anyone's prescription, call a loss of half a kilo a week safe, say a weight-reduction diet should not go below 1,000 kcal a day, and state that “approaches of rapid weight loss and use of anti-obesity drugs should be avoided”. We quote that line for completeness, not as advice about a medicine you have been prescribed: if it concerns you, discuss it with your prescribing doctor rather than deciding on your own.

This guide is food and exercise guidance alongside your doctor's treatment, not part of it, and gives no advice on starting, stopping or changing any medicine. MakeMyFitness coaches are fitness coaches, not doctors, dietitians or physiotherapists. Please read our medical disclaimer.

Why does appetite drop so much on Mounjaro, Wegovy and Ozempic?

According to the advisory, these medicines slow how fast the stomach empties, which brings bloating, fullness and nausea, and they act on brain regions that regulate appetite. It reports that they cut energy intake by 16–39% compared with placebo. Side effects are more likely in the first weeks and when the dose goes up. In the obesity trials it summarises, nausea affected 44% of people on semaglutide 2.4 mg (16% on placebo) and 28% on tirzepatide 15 mg (8% on placebo).

Eating much less has two risks. The advisory warns that such a large drop in eating can leave vitamin and mineral intake short, especially below about 1,200 kcal a day for women and 1,800 kcal for men, and names iron, calcium, magnesium, zinc and vitamins A, D, E, K, B1, B12 and C. And part of the weight lost is lean tissue: in the STEP 1 trial, 5.3 kg of the average 13.6 kg lost (38%) was lean body mass. Protein and strength training both help with that.

How do you eat enough protein when you're never hungry?

  1. 1

    Eat the protein first

The advisory suggests eating protein-rich foods first in a meal so they don't get left. Start with the dal, paneer, egg or curd, then the phulka.

  2. 2

    Eat small and often

It suggests a small breakfast, then small meals every 3–4 hours with enough fluids. Nausea often comes in the morning or after long gaps without food.

  3. 3

    Eat by the clock

If hunger never arrives, set an alarm or reminder to eat, as the advisory suggests. It says small, frequent meals may work when hunger and interest in food are low.

  4. 4

    Drink some of your protein

The advisory notes that smoothies and protein drinks with unsweetened milk or yogurt, and soups, often appeal more than heavy foods: think unsweetened lassi or a thin moong dal soup.

  5. 5

    Pick dense, soft protein

The advisory names lower-volume protein foods such as fish, eggs, Greek yogurt and cottage cheese. Hung curd is the Indian Greek yogurt, and paneer is the nearest thing to cottage cheese.

  6. 6

Sip between meals, not with them

The NHS lists a large drink with meals among the things to avoid when you feel sick. Keep a bottle nearby and drink between meals instead.

Which Indian foods give the most protein in a small portion?

With a small appetite, protein per 100 kcal matters more than protein per 100 g. Figures below are from the Indian Food Composition Tables 2017 (IFCT, energy converted from kJ) and USDA FoodData Central where IFCT has no entry. Dals and soya are listed by dry weight, before cooking.

Fish, hung curd, soya chunks and chicken breast give the most protein for the calories. Paneer still helps, but IFCT measures 24.78 g of fat per 100 g, and the advisory notes that avoiding fatty foods in the first few days of treatment can ease nausea and that high-protein or high-fat foods slow stomach emptying further. For more than 80 foods, see protein in Indian food.

What does an Indian diet plan on Mounjaro or Wegovy look like?

Here is one day at about 1,260 kcal and 84 g of protein in five small meals, vegetarian, with non-vegetarian swaps that take it to about 1,300 kcal and 105 g. It is an example, not a prescription: your calorie number belongs to your doctor or dietitian, and most men need more (recall the advisory's 1,800 kcal note).

How we counted: raw weights, the sources in the food table above, and 1 tsp of oil as 5 g at 9 kcal per gram of fat. Hung curd strained at home from full-fat curd carries more fat than the non-fat figure, so check the pack. If you can't finish a meal, eat the protein part and carry the phulka to the next one.

Can vegetarians get enough protein on Mounjaro?

Yes, with planning. In the day above, three foods do most of the work: 200 g of hung curd (21.8 g), 30 g of dry soya chunks (15.4 g) and 50 g of paneer (9.4 g). Dals add 7.2–7.3 g per 30 g dry, plus fibre. The advisory encourages plant proteins such as beans, peas and lentils alongside dairy, fish, eggs and lean poultry, with red and processed meat in moderation or minimised. For vegetarian day plans at higher calories, see the high-protein vegetarian Indian diet; for mornings, try these high-protein Indian breakfasts.

Which foods make nausea worse on weight-loss injections?

NHS advice points to fried, greasy and strong-smelling food. The NHS's general advice for feeling sick (not specific to these medicines) is to avoid hot, fried or greasy food and strong-smelling food, not to eat too quickly, not to have a large drink with meals and not to lie down soon after eating. It suggests smaller, more frequent meals, regular sips of a cold drink, and ginger or peppermint tea. The advisory adds that avoiding fatty or high-fibre foods in the first few days of treatment can help, that vomiting is more likely after large meals, and that alcohol may worsen nausea and reflux.

  • Swap fried snacks such as samosa, pakora and puri for steamed or boiled ones: idli, dhokla or boiled chana chaat.
  • Ask for less oil, butter and cream in gravies; a dry sabzi or a tikka is often easier to keep down.
  • If cooking smells set you off, eat cool foods such as curd or a cucumber salad while someone else cooks.

How much water and fibre do you need on Wegovy or Mounjaro?

Drink steadily through the day. Both US Medication Guides warn that diarrhoea, nausea and vomiting can cause dehydration that may lead to kidney problems, and say it is important to drink fluids. ICMR-NIN's general guide is about eight glasses (approximately two litres) of water a day, including other drinks, and more in hot weather and during vigorous activity. If your doctor has limited your fluids for another condition, follow that instead.

Constipation is common: in the trials the advisory summarises, it affected 24% of people on semaglutide 2.4 mg (11% on placebo) and 11% on tirzepatide 15 mg (5% on placebo). The advisory encourages fluids and fibre from foods, increased gradually, for example prunes or other dried fruit, and notes that foods high in protein or fat slow stomach emptying and can worsen constipation. The vegetarian day above has about 38 g of fibre by our arithmetic, so build up to it gradually, and ask your doctor before taking laxatives.

Is protein enough to keep muscle on weight-loss injections?

No. The advisory says increased protein alone is likely inadequate to preserve muscle without structured resistance or strength training, and that protein above what muscles need for repair or growth can be converted to fat by the liver. It states that GLP-1 medicines should be prescribed together with a structured exercise program, aiming for strength training at least three times a week plus at least 150 minutes of moderate-intensity aerobic exercise. See exercise on weight-loss injections for a 12-week plan, how we coach fat loss, and after stopping weight-loss injections.

Where does a fitness coach fit in?

A coach's job here is narrow: supervise the strength training that protein can't replace and help you reach a protein number your doctor or dietitian has agreed. MakeMyFitness is live one-to-one coaching on video across India, ₹799 a session at your own gym or ₹899 at home, and it suits people on a prescribed weight-loss injection who want strength sessions and protein-first meals planned around a smaller appetite while their doctor manages the medicine.

Our lead coach, Kumar Abhishek, holds a Diploma in Personal Training and is a Human Performance Nutritionist (K11). Our coaches are still not doctors, dietitians or physiotherapists, and Rehab & Recovery is exercise coaching only after a clinician has cleared you. See pricing and what happens in a first session.

Questions people ask

About protein on Weight-Loss Injections: An Indian Meal Plan for a Small Appetite.

  • For many adults, 100 g sits inside the 80–120 g a day that the 2025 GLP-1 nutrition advisory suggests as a practical target. The same advisory says prolonged intake at or above 2 g per kg a day should be avoided, so for a 50 kg person, 100 g is already at that limit. If you have kidney disease, ask your doctor or a dietitian before raising protein.
